Steps
File a police FIR

Download Form 26 (Application for Issue of Duplicate Certificate of Registration) from the Parivahan portal or collect it at the RTO. Fill in vehicle details exactly as they appear in insurance and PUC documents.
Submit to the RTO
Submit Form 26, FIR copy, valid insurance, PUC certificate, address proof, and vehicle chassis/engine imprint (taken on plain paper at the RTO). Pay the fee — typically ₹300–₹500 depending on vehicle type and state.
Receive duplicate RC
